Semi-Annual Monitoring Report & Annual Compliance Certification

All facilities with a final Title V Operating Permit must submit a Semi-Annual Monitoring Report by September 30 and March 31, and an Annual Compliance Certification by March 31, of each year. These reports are the basic, minimum reporting requirements under Iowa's Title V Operating Permit program. These reports allow industry, citizens, and regulatory agencies to be informed when a facility fails to meet Title V permit conditions, and what action is being taken to correct these problems.

Current DNR forms and instructions for these reports are available here for downloading. At this time, facilities are not required to use the DNR forms. However, the vast majority of facilities find it easier to use the DNR forms. If you choose to create your own forms, please carefully read the DNR instructions and review the DNR forms to be sure you include all of the required information.

The DNR forms are available in both PDF and Word format. If you plan to use the Word format, we recommend that you also download, save, and store a clean copy of the forms in PDF format, as well.
