For your convenience, DNR launched a new online payment portal for facilities to pay annual Title V fees. Go to https://programs.iowadnr.gov/payments/ and search for the facility by emission inventory questionnaire (EIQ) number, facility identification number, or facility name as it appears in the State & Local Emissions Inventory System (SLEIS). Payments may be made by credit card or electronic check. No login is required. Use the payment portal and there is no need to send in the Form 5.0.
If submitting by paper, please include the Form 5.0 along with the payment. No signature is required on the form. Please send the Form 5.0 and payment to:
