The DNR is proposing to implement a new air quality operating (Title V) permit application form – EZ Mod Form. The new EZ Mod Form is an optional alternative to the standard Title V application forms to request a minor Title V permit modification. To reduce submitting duplicate information, this form incorporates data elements from air construction permit applications and final issued air construction permits.
The current draft form and associated guidance are based on suggestions from the regulated community and also DNR staff. The DNR is providing the public with an opportunity to comment on the new form and guidance. Comments on either the technical aspect or the form formatting, layout and guidance are welcome.
