The DNR has a new air quality operating (Title V) permit application form – EZ Mod Form. The new EZ Mod Form is an optional alternative to the standard Title V application forms to request a minor Title V permit modification. To reduce submitting duplicate information, this form incorporates data elements from air construction permit applications and final issued air construction permits.
The new form is an easy and efficient way to get minor permit changes incorporated into a Title V permit and is based on suggestions from the regulated community. The EZ Mod form and guidance are available under Title V Forms & Instructions on the Air Quality Bureau’s Operating Permits website at https://www.iowadnr.gov/AirOperatingPermits.
